Services offered
Dr Chan accepts referrals for general and ongoing psychiatric care, as well as one-off assessments and opinion (Item 291) . She has particular interests in psychotherapy, mood disorders and trauma-related disorders. She sees adults and young people from age 16 years.
Referral information
An email or secure message referral is preferred. Please include in the referral letter:
- Type of service being requested, particularly in the case of assessment and report (item 291).
- Patient’s name, address, date of birth, contact numbers, email and Medicare details
- Outline of presenting problem
- Previous mental health history, treatments and potential risk factors
- Relevant medical history
- Current medications
- Other health care providers involved in the person's treatment
