For Referrers

Dr Chan accepts referrals for general and ongoing psychiatric care, as well as one-off assessments and opinion (Item 291) . She has particular interests in psychotherapy, mood disorders and trauma. She sees adults and young people from age 14 years.

An email or Argus referral is preferred. Please include in the referral letter:

• Type of service being requested, particularly in the case of assessment and report (item 291).
• Patient’s name, address, date of birth, contact numbers, email and Medicare details
• Outline of presenting problem
• Previous mental health history, treatments and potential risk factors
• Relevant medical history
• Current medications
• Other health care providers involved in the person's treatment
